Diabetes Self-Management Education

For:
- Newly diagnosed patients
- Uncontrolled Type 1 or Type 2 diabetics who have never had a formal self-management class
Program features:
- An individual assessment prior to beginning program
- Three classes utilizing ADA guidelines for the curriculum
- Afternoon and evening classes available to meet your needs. We even offer a “Diabetes in a Day” Class!
We also offer:
- Gestational diabetes classes
- Diabetes support group
The Longstreet Clinic Diabetes Education Program is provided by a team that includes a Certified Diabetes Nurse Educator and Registered Dietitians utilizing ADA guidelines for the curriculum. A physician referral may be required.
Dr. Philip Marler, Dr. Lajos Toth and Dr. Kenneth Fogarty, Internists practicing at The Longstreet Clinic, are recognized by the NCQA & ADA through the Diabetes Physician Recognition Program, as are Dr. Marti Gibbs and Dr. Andrew Reisman with TLC's Department of Family Medicine in Oakwood.
