The patient-centered medical home (PCMH) is a model of care that puts patients at the forefront of care. PCMHs build better relationships between patients and their clinical care teams. Practices that earn recognition have made a commitment to continuous quality improvement and a patient-centered approach to care.
NCQA’s Patient-Centered Medical Home (PCMH) Recognition program is the most widely adopted PCMH evaluation program in the country. More than 13,000 practices (with 50,000+ clinicians) are recognized by NCQA.
Research shows that PCMHs improve the patient experience and quality, as well as increase staff satisfaction — all while reducing health care costs.
The NCQA Patient-Centered Medical Home comprises six concept areas:
- Team-Based Care and Practice Organization: Helps structure a practice’s leadership, care team responsibilities and how the practice partners with patients, families and caregivers.
- Knowing and Managing Your Patients: Sets standards for data collection, medication reconciliation, evidence-based clinical decision support and other activities.
- Patient-Centered Access and Continuity: Guides practices to provide patients with convenient access to clinical advice and helps ensure continuity of care.
- Care Management and Support: Enables clinicians to set up care management protocols to identify patients who may need more closely-managed care.
- Care Coordination and Care Transitions: Ensures primary and specialty care clinicians are effectively sharing information and managing patient referrals.
- Performance Measurement and Quality Improvement: Helps practices develop ways to measure performance, set goals and develop activities to improve performance.
According to the NCQA, “a growing body of scientific evidence shows that PCMHs are saving money by reducing hospital and emergency department visits, mitigating health disparities, and improving patient outcomes.”
